HOW TO APPLY

Once you decide which category/categories you wish to apply for, submit the Application Form along with the following:-

1. Statement of Purpose (SOP)
   Each entry ought to send a SOP of around five hundred - one thousand words containing a quick description of yourself, your passion compassion, your practice/case/category that will highlight the explanations you think that you must win that class.

2. Photos & Presentation
   Ensure that your entry includes pictures to enable the judges to differentiate your entry from others. Photographic proof for the category entered should be sent along with textual matter in CD/DVD/Pen drive or may be emailed via google drive.

WHY TO APPLY

• Showcase your achievements on social media. A Nomination or Winning an award gives not only professional boost but sense of achievement for your passion and most importantly satisfaction.
• Trophy to adorn the practice and highlight your success. More patients & better staff are attracted to an award winning practice.
• Nominees and winners will be permissible to use the award brand on their web site, brochures, posters or the other selling material.
• Nominal Entry Fees is charged to cover logistics.
• Nominees & winners will receive Nationwide exposure through the multiple newspapers, news channels, social media, print media,  journal publications & various website.
